Privacy Policy

I understand that the records concerning services will be retained. Such data will be kept confidential according to the California Welfare Code Section 5328.  No information will be released to agencies or persons outside of the county/agency mental health system without my written consent, except in the case of a medical emergency. 

What is discussed in sessions is confidential unless and until I give my consent to its release, with two exceptions: (1) Consultant will need and is mandated by law to inform, if they hear and believe that my child (I) is (am) in danger of hurting himself/herself (myself) or someone else and (2) if there is a reasonable suspicion that a child, dependent adult, and/or elderly adult has been abused.    

I have read the foregoing, understanding its content and agree to the conditions stipulated therein.  I hereby authorize WH Consulting to provide services as agreed upon.
