WH Consulting is an intensive parent education service. For best outcomes to occur, the model is designed with a consistent appointment schedule in mind.

I would like to be a voluntary individual with WH Consulting which is a consulting service.  The conditions of the services have been explained to me to my satisfaction.  I agree to participate in Consulting Services.

• Services in the office or family home are generally scheduled for a minimum of one time per week, but frequency can be increased or decreased depending on the need of the family.

• Service provider respectfully requests 24 hours or more notice to cancel appointments whenever possible. When this does not occur, the regularly hourly charge will be invoiced.

• If there are extenuating circumstances such as illness or family emergencies, a fee will not be invoiced.
